This is very similar to modern earth moving machinary used. Instead of steam, Hydraulic (high pressure oil) is used. The crank shaft would transfer power to worm which would then rotate wheel gear (middle right)rotating the whole assembly over thrust bearing. The MG Nilagiri Express between Mettupapaiyam and Ooty used to run till a few decades ago with number 35/ 36.
